Instructions For Filling Out Application 1. You require Adobe Acrobat to fill out the application form. If you don't have the latest version click here for free download. 2. Download the Local Live-in Caregiver Application Form. 3. When you are finished filling out the application, send it to CFNS. If you want to work in Calgary, select the "Send Application to Calgary" button. If you want to work in Edmonton select the "Send Application to Edmonton" button. If you want to work in Toronto or Greater Toronto Area select the "Send Application to Toronto/GTA" button. Print a copy for your records. Employment Contract (next page)
